This software is designed to collect survey material from the participants. A variety of indicators are designed to allow the user to express their response in a quantitative and qualitative manner. As seen in the slides, dropdown options allow the user to answer questions related to their previous answer. The slider option allows the user to indicate a level of pain without facing the anxiety of numbering their pain. This software is carefully designed to receive the most amount of information from the participants in the least amount of time.
